To register

We will ask you for proof of your current address as well as photographic ID to verify that your details are correct.

If you do not have a printer please visit reception from 8am to 6pm to collect these forms.

Visitors / Temporary Residents

Patients who are visiting and staying in the Practice area may be seen at the Practice as Temporary Residents.

Anyone requiring to be seen by a doctor should attend the Practice and complete the necessary forms to allow the reception staff to temporarily register you on the Practice computer system.

Once your details have been recorded you will be able to see a doctor for a consultation.  This will generally mean that you will have to wait until the next available surgery session before being seen.

Visitors from Overseas will be asked to provide proof of identity.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
