Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else.  

With the introduction of new data protection laws (GDPR) we are required to have your permission if you wish for a third party to collect any items (e.g. prescription, letters, blood requests) on your behalf or for a third party to discuss your medical care.  If you would like to give access to a third party please collect a form from reception. 

Before we can hand over or disclose any information our staff will ask for personal identification.
