Gullane Flu Clinics - UPDATE


We will be running flu clinics here at the Practice for patients over the age of 65 *Patients aged 6 months to 18 years with a chronic disease should also book into these designated clinics.

THURSDAY CLINICS ARE NOW AVAILABLE


These will be pre bookable clinics, please phone a member of the reception team on 01620 842171 to book one of these appointments. You will be assigned to a specific Health Care Professional at a specific time, it is important that you arrive at the time given. When you attend we ask that you observe social distancing and wear a face covering. Please do not attend if you do not have an appointment.



CHILDREN AGED 2-5 YEARS OLD WILL RECEIVE A FURTHER LETTER FROM COMMUNITY CHILD HEALTH WITH AN APPOINTMENT DATE, TIME AND VENUE. THESE WILL NOT BE BOOKED OR ORGANISED THROUGH THE PRACTICE.


 PATIENTS AGED BETWEEN 18-64 YEARS OLD WITH A CHRONIC DISEASE WHO HAVE RECEIVED A LETTER FROM THE HEALTH AND SOCIAL CARE PARTNERSHIP WITH REGARDS TO  FLU VACCINE. FOR THIS COHORT OF PATIENTS WE ARE AWAITING FURTHER CLINICS TO BE ORGANISED BY THE HEALTH AND SOCIAL CARE PARTNERSHIP (HSCP) - KEEP AN EYE ON OUR WEBSITE FOR FURTHER INFORMATION AND ADDITIONAL CLINIC DATES AND TIMES

New Patient Registration

registrationIn order to register with the Practice you should complete a registration form that you can either pick up from surgery or download from website. Bring it along to us together with ID showing your current address as well as photographic ID.   

Practice Area

The Practice serves the area between the boundaries of, but excluding North Berwick and Longniddry. Due to growing patient numbers and demand we cannot accept any new patients from North Berwick or Longniddry.


Registration Form

You can download the PDF version of our registration form below.  Please complete this and bring it to the surgery together with ID showing your new address and also photographic ID ie passport, drivers licence.  This process needs to be completed before you need to see a doctor or nurse so please make sure we have the forms as soon as possible.  We would be grateful if you could also complete the Medical Questionnaire and let us know if you are someone who is being cared for and who your carer is, or if you are a carer for someone else.
